The Clinical Real-Time Dilemma
Building a video consultation portal for healthcare providers is fundamentally different from building a generic chat application. Outpatient clinics and remote patients face unique challenges:
- Hospital Symmetric NAT Firewalls: Medical clinics operate on locked-down corporate intranets that actively block direct peer-to-peer UDP traffic.
- Bandwidth Volatility: Patients in rural locations connect via 3G/4G connections with frequent packet drops.
- Strict Compliance & Privacy: Video streams and patient health records must never pass through untrusted intermediaries without end-to-end encryption.
- Interactive Workflow Integration: Video alone is insufficient; doctors need to write prescriptions, annotate diagnostic imaging, and issue lab slips in real-time.
WebRTC Connection Architecture & Signaling
WebRTC enables browser-to-browser audio and video streaming with sub-200ms latency. However, browsers cannot connect directly without an initial handshake protocol:
- The Signaling Channel: A Node.js server using Socket.io exchanges Session Description Protocol (SDP) offers, answers, and ICE candidate packets between the doctor and patient.
- STUN (Session Traversal Utilities for NAT): Allows clients to discover their public IP address and port mapping. Works for 80% of standard home networks.
- TURN (Traversal Using Relays around NAT): When strict hospital symmetric NATs prevent direct peer connection, traffic relays securely through an encrypted TURN server (such as Coturn).
Real-Time Digital Prescription Canvas Sync
During a remote telehealth consultation, doctors draft treatment plans while observing the patient. Rather than relying on separate software, we architected an integrated digital prescription canvas:
- State Synchronization: Doctor keystrokes and selected medications are streamed via low-latency Socket.io rooms directly to the patient's companion view.
- Cryptographic Signature Verification: Upon consultation completion, the doctor enters their PIN to generate an HMAC-SHA256 signature stamped onto the generated PDF.
- Automated Pharmacy Dispatch: The prescription PDF is encrypted with AES-256 and dispatched via automated WhatsApp webhooks to partner pharmacies with verifiable QR verification codes.
HIPAA & Data Protection Checklist
To ensure healthcare platform compliance: - Enforce DTLS-SRTP encryption on all WebRTC media streams. - Ensure zero temporary storage of unencrypted video frames on servers. - Maintain immutable access audit logs for all medical history queries. - Store patient demographic records and EHR files in encrypted volumes with role-based authorization.
