The Challenge & Engineering Friction

In acute medical emergencies and trauma care, finding compatible, verified blood donors often suffers from severe communication latency and fragmented records. Regional blood banks traditionally operate in disconnected silos with un-synchronized unit counts, while voluntary donor directories either leak donor personal phone numbers to public scrapers or rely on stale, unverified contact lists. This friction results in life-threatening delays during critical transfusion windows. Furthermore, prospective donors are reluctant to register on public registries due to privacy infringement concerns and spam calls. Healthcare coordinators and donation camp organizers also lack a unified, role-governed management system to orchestrate blood drives, publish urgent hospital requisitions, and monitor real-time stock levels across all eight primary blood groups (A±, B±, AB±, O±) with strict data integrity.

The Technical Solution & Architecture

ARdevlopers engineered an enterprise-grade, privacy-centric blood donation and inventory platform utilizing Node.js, Express.js, MongoDB, and Handlebars server-rendered views. To protect donor privacy while facilitating rapid emergency response, the platform features a two-way contact permission handshake: donor phone numbers remain strictly masked during search queries until a requester initiates an authorized contact request that the donor explicitly reviews and approves from their profile console. The system incorporates granular Role-Based Access Control (RBAC) separating Donors, Blood Bank Inventory Managers, Admins, and Super-Admins. It integrates the Twilio Programmable Messaging API for time-expiring One-Time Password (OTP) verification, distributed session persistence via connect-mongo, and Server-Sent Events (SSE) connections to push real-time browser alerts when contact requests are approved or urgent patient requisitions are published.

Core Architectural Decisions

Data Persistence & Session State Layer

Built on MongoDB with Mongoose ODM modeling structured collections for Donors, Blood Banks, Donation Camps, and Contact Handshakes. High-concurrency session persistence is decoupled using connect-mongo backed by dedicated MongoDB session stores to eliminate session loss across server restarts.

Event-Driven Real-Time Notification Pipeline

Implements persistent Server-Sent Events (SSE) over HTTP streaming with an in-memory client connection pool. Triggers asynchronous push notifications for contact request state mutations and approvals without incurring client-side polling overhead.

Access Control, Telephony & Cryptographic Security

Enforces tiered route middleware across public, donor-authenticated, blood-bank-authenticated, and super-admin tiers. Uses bcrypt (10 rounds) for credential hashing and integrates Twilio REST API for secure E.164 phone normalization and time-bound OTP generation.

Key Engineering Features & Capabilities

Privacy-First Contact Handshake: Masked donor contact information in search results with a secure request-and-approval workflow protecting donor PII.
Real-Time Event Notification Engine: Server-Sent Events (SSE) streaming architecture that pushes instant status updates and contact approvals directly to active users without polling.
Multi-Branch Blood Bank Inventory Ledger: Dedicated staff portal enabling blood banks to update unit availability across all 8 blood groups (A+, A-, B+, B-, AB+, AB-, O+, O-) with automated timestamps.
Emergency Requisition Board: Urgent hospital patient requisition feed allowing verified users to broadcast immediate blood shortages with hospital details and required unit counts.
Two-Factor SMS Verification & RBAC: Twilio-integrated 6-digit OTP verification for password resets, bcrypt password hashing, and multi-tier administrative governance (Super-Admin, Admin, Blood Bank, Donor).
